For almost 25 years, USDAA has recognized and honored individuals who have contributed to the development and growth of dog agility. Starting in 2014, inductees to the USDAA Hall of Fame will be announced annually at the Cynosport® World Games.
All 2014 nominations must be submitted by 5 p.m. MST, September 1, 2014 for consideration. Send nominations to email@example.com
1. Remember to include the nomination form.
a. Be sure to fill out all the contact information so we can contact you if we need anything clarified.
b. Check the appropriate nomination category. If nominating the same person or group in more than one category, please complete a separate nomination form for each category.
c. If nominating for the Years of Service award, remember to check the box for the number of years.
2. A short essay (500 words or less) is required on how this nominee meets the criteria for the category selected and must include the following information:
a. The person or group's name
b. Their hometown and state
c. Current profession
d. Birth year
e. Number of dogs
f. Daytime contact info
g. Email address
3. You can also include up to two photos with your submission. Please note - If you are submitting a photo or other images, you are granting USDAA, its successors, agents or assigns a non-revocable and non-exclusive right to use these photos or images without royalty at any time. You also indemnify USDAA from any claims or losses from claims of copyright infringement that may arise.
4. There are no restrictions on the number of inductions into USDAA's Hall of Fame for any category listed.
Once all nominations are received, they will be reviewed and categorized by USDAA staff. If necessary, a USDAA staff member will contact you for additional details. All nominations will be voted on and approved by the USDAA Advisory Board.
Inductees into the USDAA Hall of Fame will be announced at the 2014 Cynosport® World Games in Morgan Hill, California, October 15-19, 2014. Specific dates and times for the award ceremony will be determined as we finalize details of the event.
