  • Event Administration Kit
    • ERB Summary Page - This is an Excel spreadsheet that can be downloaded and used to summarize the qualifications by class.  This form should be included with the submission of the event ERBs.
    • Organizing Committee Report - Groups use this form to report on a sanctioned event following conclusion of the event.
    • Disciplinary Action Complaint Form - Disciplinary Action Committee Chairs should use this form to report incidents or complaints at an event which do or do not result in a committee hearing.  If a hearing is held, decisions rendered by the committee shall be reported using this form.  Guidelines found in Appendix C of the USDAA Rulebook apply.
    • Official Approved Move Up Forms - Find both the day-to-day and 24-hours waiting period forms here.
    • Injury Reporting Form - Use this form to report injuries to dogs, whether in competition, training or arising from other activities with possible links to performance in dog agility.
    • IRS Form W-9 - Request for Taxpayer Identification Number - This IRS form is used to request a taxpayer identification number for purposes of reporting nonemployee compensation. A group may need this form for payment to judges, seminar instructors and others to whom they make payments for services. Backup withholding and special a special filing may be required. Consult your tax advisor or the IRS web site at www.irs.gov.
    • USDAA Marketing Toolkit - A public relations toolkit for clubs and competitors to help promote events and achievements with your local media.
    • USDAA Logos - USDAA logo specifications and designs for use by USDAA Affiliated Groups for awards and web site presentation. The USDAA lettering mark is a separate logo from the three weaving dogs crescent mark.  They may be used separately or together.  The text DOG AGILITY is not part of the logo and is optional, but is recommended to be used when readers are not familiar with USDAA.
